The template works with both the Microsoft Store edition and the desktop edition of the app. Make sure you have installed one of them.
How to use:
This form was created as a sample of customization. The base template is cn003.
On the top of the form, there is a section for vendor information. It contains a line for "Federal Tax I.D. Number". This could be a FEIN or SS#. In the "Bill To" section, we reuse the original "Client#" and title it "DOR Number". If you have both a customer number and a DOR authorization number to be included on your invoices, simply add a custom field for the DOR number.
We also added three columns as compared to the original template, i.e. "Consumer Name", "Authorization #Number" and "Service Date".
The "Signature" line on the bottom of the form is not a custom field. It is there for handwriting signatures. This means anything you type in the field will not be saved.
The line "California Sales Tax" is an additional line. Making it a separate field enables you to add it to reports as a separate column. The default tax rate is set to 6.5%. To change your tax rate, click "Settings", and then go to the "Taxes" tab.
