How to use:
This template features a number of custom fields that are added specially for vendors for the ADP Assistive Devices Program (ADP).
The Assistive Devices Program (ADP) is a beneficial program for anyone needing to access Government assistance for pressure modification devices (compression garments). This Canadian ADP invoice template includes fields for "ADP Claim #", "Health Card #", "Serial #", and "Benefit Code". All these are defined as custom fields, so they can be saved to the backend database when you click "Save To DB".
This Excel form has 8 columns in the detail section: "Product ID", "Side", "Description", "Qty", "Unit Price", "Extended Price" (usually called "Line total" or "Amount"), "Client", and "ADP". The "ADP" column is calculated automatically based on "Extended Price" and "Client", using an Excel formula like this:
= oknLinetotal_? - oknClient_?
Note that the question mark here indicates the line number in the detail section.
This template has one tax set by default ("HST"). See Canadian Invoice Template with HST (c6020) for another example on this topic. If you need to change the tax calculations, click the "Settings" button on the ribbon tab, and then go to the "Taxes" tab.
