This form features a staged image as the background. It is able to charge different tax rates on materials and labor.
The template offers both a "Bill To" section and a "Project Location" section. If the "Bill To" address is the same as the "Project Location", you can fill in the "Project Location" fields easily by clicking the "Same As Bill To" button.
This template is a modified version of Mixed Tax Rates (5 Columns) (c5034). It offers an "Item Type" (a drop-down list) and "Applied Tax Rate" option (a percentage value) for each item.
The "Item Type" and "Applied Tax Rate" cells are outside of the printable area. If you want to show them in the printable section, visit the 7-Column version (c5035).
