This template is designed for junk removal service businesses. It features a carton image as a watermark at the bottom of the sheet. It shows a dustman picking up a bag of trash and putting it into a trash can. It is suitable for both freelancers and small businesses in the industry.
It is based on the Labor Invoice template (c4070). In the original version, the columns are titled "Hours" and "Rate". This template changes the column titles to "QTY" and "PRICE". The Excel formulas that calculate the line totals remain the same. It looks like this:
=ROUND(oknQuantity_1*oknPrice_1,2)
Note that the "Discount" implemented at the bottom of the form is manually editable. It is an amount, but not a percentage. The "Total" is calculated using the formula like this:
=ROUND(oknSubTotal -oknDiscount + oknShippingCost + IF(oknTaxType=0,0,IF(oknTaxType=1,oknTax1,oknTax1+oknTax2)),2)
This is a deeply customized form. If you prefer a simple format, try the simple invoice format (c4032).
