The template works with both the Microsoft Store edition and the desktop edition of the app. Make sure you have installed one of them.
How to use:
This form is suitable for general hospitals, pharmacies, clinics, and home health agencies. It shows the amount of expenses for medical services or treatment.
On the top of the spreadsheet, you'll first notice there is a logo image showing "BoostExcel", which is a placeholder for your own logo image. Near the logo image is your business information.
This form is intended to be as general as possible. In the "Patient" section, you'll find fields for Hospital No., Name, Address, Phone, Email, Patient Age, Consultant, Payment Mode, Bed No., Admission Date, and Discharge Date.
The detail section includes 5 columns: #, PARTICULARS, QTY, RATE, and AMOUNT.
At the bottom of the form, there is one tax. Another custom field, Med Claim, is also added. The "Total" is calculated with a formula like this:
subtotal + tax - Med Claim
