Psychologist Invoice Template

The template works with both the Microsoft Store edition and the desktop edition of the app. Make sure you have installed one of them.

How to use:

  1. Download the EXE package.
  2. Double-click to extract the files into a folder. Make sure you have full access privileges to the folder, such as a subfolder inside “My Documents”.
  3. Open the template.

This form is designed for psychological services. It features a "Bill To" section, a separate "Patient" section, and six columns: "Date", "Service", "Charges", "Payments", "Insurance Payments", and "Current Balance".

If you are a psychologist, whether clinical or counseling, this template will work for you. It is designed as a progressive work, job, and payment statement. With this understanding, the form is also suitable for creating statements and bills for other business types with small modifications, such as large projects that should be done and get paid in several phases; cleaning service (c4041) or lawn care service (c4065) that should be done regularly; or freelance service (c6018) rendered multiple times; in these cases, the "Patient" section could be replaced with something like "Project Description", "Job Description",  "Service Description", etc.

The "Service" and "Charges" columns are called "Description" and "Price" columns with the usual templates. The "Quantity" column is moved out of the printable area and assigned a formula so that if the "Service" column is not empty, the "Quantity" column is assigned 1.

Except for the "Current Balance" column, which is in fact the "Line Total" column in the usual templates, all the other columns are manually fillable. The calculation of "Current Balance" is done by a formula like this:

= Charge - Payments - Insurance Payments

As you can see, we completely redefined the calculation method of the "Current Balance" or "Line Total" cells.
